Attention: 2021/2022 [4 year degree] students of the CS/IS/SE programmes
Dear student,
All students of the current 4 Year Honours Degree [2021/ 2022] are requested to fill out the relevant form below by uploading requested documents on or before 19.10.2026 to verify their name to be written on the degree certificate.
Failure to submit the duly completed form by the stipulated deadline will result in the degree certificate not being prepared.
The name appearing on the degree certificate will not be altered or amended under any circumstances once the certificate has been issued.
Note:
Please note the above forms are applicable only to students of the four year degree programme of 2021/22 intake. The forms relevant to students of the three year degree programme (2022/2023 intake) were collected at the time of enrolment. Therefore, students of the three year degree programme who have made any changes to their full name after enrolment are required to inform the Examination Division in person on or before 19.10.2026.
